Healthy Hearts Cardiovascular Disease Prevention Service

Healthy Hearts is the new Cardiovascular Disease Prevention Service for the London Boroughs of Hammersmith & Fulham, Kensington & Chelsea and Westminster with a major goal to reduce local residents' CVD risk by assisting them to lose 5% body fat, by engaging them in a variety of intervention such as 10 week lose weight with Health Hearts, 6 weeks cook ad eat course, 6 week physical activity classes ( begin to swim, begin to gym, aquaerobics and more), plus smoking cessation with Kick-It and Psychology input if required.
The service is for residents who meet the following criteria:
- Assessed in a Health Check as having a risk of cardiovascular disease over the next 10 years
- Diabetic or another long term condition that increases the risk of CVD
- Obesity (BMI over 30)
Clients are assessed and then they discuss with a Health Hearts Care Planner to advise what the best intervention would be suitable for them.
