Westminster Learning Disability Team
The Westminster Learning Disability Team is part of the Westminster Adult Social Care Department.
The team works with people with learning disabilities aged 18 and over and their family carers to identify where support is needed and how this can be provided in line with an individual’s person-centred plan.
Working closely with Children’s Services, the team helps support young people in transition as they move from childhood into adulthood and are perhaps moving from Children’s to Adult’s Services.
Together, Children’s and Adult Services will share information and consult with the young person about their education, health and social care needs, as well as their goals and future employment opportunities. During this stage, the young person may wish to find out more information about direct payments and personal budgets to supports their level of independence and their ability to access other services.
The team is the first point of contact for anyone with a learning disability and their family/carers who would like support or just some information and advise.
The Westminster Learning Disability Team wants people with a learning disability to have the same choice and control in their lives as everyone else in Westminster.
